Engagement: Part-Time Advisory Board Member for Diabetes Prevention Startup

Fruit Street Health is a CDC-recognized diabetes prevention program designed to help patients with prediabetes prevent progression to Type 2 diabetes through diet and lifestyle changes. The program is delivered via telehealth and live group Zoom sessions, led by registered dietitians, and offers a comprehensive, interactive approach to chronic disease prevention.

Role and Responsibilities

We are seeking family medicine physicians to serve as advisory board members for Fruit Street Health. In this role, you will provide strategic guidance on the clinical direction and implementation of our Diabetes Prevention Program (DPP), with a focus on integrating it into primary care settings and population health initiatives. You will help shape best practices and support program adoption across diverse patient populations, including Medicare, Medicaid, and commercially insured patients.

• Advocate for the program within your physician network and encourage referrals of patients with prediabetes
• Advise on integrating the DPP into primary care workflows to improve accessibility, outcomes, and patient engagement
• Provide clinical insights to refine program guidelines and implementation strategies for broad patient populations
• Engage with healthcare executives and payers to support program adoption in primary care, health systems, and value-based care models
• Network with employers, health plans, and state health departments to promote the program and expand access across communities
• Attend major conferences such as AAFP FMX (Family Medicine Experience) on behalf of Fruit Street, with a travel stipend provided

Qualifications

• Board-certified family medicine physician (MD/DO)
• Experience in chronic disease prevention, public health, or population health preferred
• Strong interest in digital health innovation and telemedicine
• Passion for preventative care and improving health outcomes across diverse populations
