As a valued RN or LPN at Golden Heart Advocacy, you will take the lead in managing and overseeing essential medical supports for individuals with health-related needs, while equipping and mentoring our team to ensure safe, effective, and compassionate care. Your purpose is to uphold the highest standards of health, safety, and dignity always guided by empathy and respect.
Key Responsibilities:
Develop Personalized Care Plans Tailor nursing care plans that reflect the unique medical needs of each individual we support.
Perform Health Assessments Monitor health status, manage chronic conditions, and respond to emerging concerns with clinical insight and care.
Train & Support Staff Educate Direct Support Professionals (DSPs) on:
Dietary needs & safe feeding practices
Mobility and transfer techniques
Medication Administration- Once certified
Use of EpiPens, enemas, injections, suctioning, and other medical tasks (within scope of practice and regulation)
Ensure Compliance & Quality Maintain up-to-date knowledge of DDD, Medicaid, and state nursing regulations to ensure all care and training meets or exceeds standards.
Document with Excellence Complete timely and accurate records of nursing services, health interventions, incident reports, and progress notes and address advanced needs forms.
Collaborate Across Teams Work closely with families, support coordinators, and interdisciplinary teams to ensure a unified approach to care. Participate in intake meetings and provide clinical insight for individuals with higher medical acuity.
Who You Are:
A licensed RN or LPN
Experienced in supporting individuals with developmental or intellectual disabilities (preferred, but not required).
Confident in training others and simplifying medical information for non-clinical staff.
Detail-oriented, compassionate, and proactive in your approach to care.
A strong communicator and natural team collaborator.
